Self Referral Process

Start directly, without losing clinical context.

The self-referral pathway is designed to collect enough information for the scan request, safety considerations and next steps to be reviewed appropriately.

How self-referral works

A direct route should still be structured and clinically responsible.

A self-referral request typically includes the body area being discussed, symptoms, relevant history, previous imaging where useful, GP or clinician details and appointment availability.

Self-referral does not mean choosing a scan completely without review. The information you provide can help the imaging team confirm what needs to happen before an appointment and whether additional clinical input is required.

The journey

Five stages, explained before you submit anything.

Timing can vary by examination and availability. The booking information should explain the expected appointment and reporting pathway for your request.

01

Submit your details

Provide your contact information, body area/scan request, symptoms and any relevant previous imaging or referral documents.

02

Clinical and safety review

The information is reviewed to clarify the imaging question and identify any MRI safety or suitability issues that need to be discussed.

03

Appointment confirmation

Once the appropriate route is clear, you receive appointment information and any preparation instructions relevant to the scan.

04

Attend your MRI

The radiography team confirms the safety information, explains positioning and supports you through the scan.

05

Report and follow-up

The report and images follow the agreed delivery pathway. Medical interpretation and treatment decisions should be discussed with an appropriate healthcare professional.

Self-referral form

The right detail helps avoid unnecessary back-and-forth.

Provide the clinical context that helps the team understand your request and assess the appropriate next step.

Body area, side, symptoms, duration, relevant history and previous imaging when available.

About implants, metal, previous surgery, pregnancy or anything relevant to MRI safety.

Health information should be shared only through the appropriate booking or referral route and handled securely.
